Primary & Preventative Care

It’s old advice, but true: see your doctor. Routine care keeps people healthy. Visits to the Harbor Care Health & Wellness Center help you and your family handle or stop illnesses before they get too serious. Regardless of your income or insurance status, you are welcome at Harbor Care.

What to Expect at Your Visit

You will meet with our team—doctors, nurses, and other providers—and together we will help develop a plan for your regular care. Our PHYSICAL health care services include:

  • Yearly wellness physical exams.

  • Comprehensive family services.

  • Pediatrics.

  • Diagnosis & treatment of minor illness.

  • Medication management.

  • Asthma management.

  • Diabetes management.

  • High blood pressure management.

  • Screenings (e.g., blood sugar, blood pressure, cholesterol).

Our women’s health services include:

  • Well-women care/gynecology.

  • Prenatal/postnatal care.

  • Family planning services.

At your first visit, you will need to fill out paperwork to make sure we have your medical history. This allows us to give you the best care possible. Patient Portal

Visit our new Patient Portal - a secure online website that gives patients convenient, 24-hour access to personal health information from anywhere with an Internet connection. Using a secure username and password, you will be able to view your health information, including appointments, payment options, and medications. Learn all about the Portal with the FAQs here.

Cost & Insurance | Sliding-Fee Scale

Thanks to the support of many donors and funders, we offer care to all those in need, including those without insurance or the ability to pay for services, using a sliding fee scale based on income. We accept most insurances, including Medicaid, Medicare, and private insurance. Please contact us with any questions about insurance or payment, or to schedule an appointment with our Patient Insurance Navigator, who can help you understand and apply for any insurance you may be eligible to receive.

 Mental Health Care

Mental health matters. We pride ourselves in serving all community members who need it through one-on-one, group and family counseling, as well as psychiatry and medication management. We serve individuals, couples, families, veterans, and children. If you or someone you know is struggling with substance use, we also provide substance use treatment. To learn more please contact us

What to Expect at Your Visit

During your first visit, you will meet one-on-one with a clinician by phone, web, or in person, to discuss your needs and develop a care plan together. These sessions are typically about an hour. 

Care plans are unique to you. They may include routine visits with counselors or a psychiatrist and strategies to curb symptoms, such as medication. To the maximum extent possible, we want to help you feel: 

  • Well.

  • Able to work.

  • Able to take care for yourself and you family.

  • That you understand your symptoms.

  • Able to cope with stress, depression, anger, or other symptoms.

Appointments are scheduled, but same-day and crisis appointments are available. Please call before visiting for special instructions.

ONE-STOP CARE: Pick up your medications right after your doctor visit.

The Harbor Care Health and Wellness Center Pharmacy is here to serve with compassion, care, and human dignity. Whether it is filling prescriptions, helping you to understand how to take a medication, or managing medication side effects, our professional pharmacy team enthusiastically takes care of our patients’ healthcare needs. Because we are located on-site within the main Harbor Care Health & Wellness Center location, we are able to work closely with you, your doctors and clinicians, and other members of your care team quickly and efficiently.  All profits of our pharmacy are invested back into the services offered through Harbor Care. (Pharmacy services available for Harbor Care Health & Wellness Center patients only.)

Insurance & Costs

As the only nonprofit pharmacy in Nashua, we are able to serve our uninsured clients with substantially lower prices through a sliding-fee scale (right) based on a person or household’s income. We accept most insurance plans, including Medicare Part D.

Our Community Health Services comprise frontline staff who focus on improving individual and community health while providing access to healthcare and social services that support well-being.

Harbor Care’s Community Health Workers are trusted members of the community who connect clients to vital services, partner with social service providers, provide health education information, and create supports designed to help individuals navigate New Hampshire’s social services system while addressing barriers to services.

Outreach

Our Community Health Workers largely operate in the field, offering a low-barrier solution to many healthcare needs. They attend local events, collaborate with community organizations and Harbor Care programs, and help individuals gain access to healthcare and services they may not even be aware exist.

Mobile Health Van Service

Our mobile health van service is coming to your area! Click here for the latest schedule. Services offered include telehealth for established patients, health insurance navigation, and connection to all Harbor Care services. Current locations throughout the Nashua, Milford, Merrimack, and Hudson areas.

Connecting to Resources

In addition, Community Health Worker staff help individuals and families accessing health and wellness services through Harbor Care, assist individuals who need to apply for social service programs, and provide referrals to local organizations that could help people improve their daily lives and overall well-being.

Educate

Harbor Care Community Health Worker staff increase the public’s awareness of chronic diseases, prevention, risk factors, nutrition, and health management. The healthcare and social service systems can be complex, and our Community Health Services team help individuals understand health information, make goals, and advocate for themselves and their communities.

Health Insurance Assistance

Harbor Care’s Community Health Insurance Navigator is a trusted individual embedded in our Community Health Services program who helps educate and inform the public and provides assistance in navigating health insurance options through Medicaid, CHIP, and Federal Health Marketplace.

Our Community Health Insurance Navigator helps individuals enroll and follow up to finalize plan options and healthcare services to help improve health and overall well-being. For an appointment, call 603-821-7788.
